Aultman Health Foundation
The Health and Wellness RN role is a part time position that will be scheduled four days a week. The position will be located primarily in North Canton, Ohio.
PURPOSE OF POSITION:
The purpose of the Health and Wellness Center is to facilitate a culture of health and wellness in an employer group setting. The RN will complete biometric screenings, educate on the results and motivate members to become engaged in their health care. Education will be done on an individual basis, in a group setting or as a presentation to a large group.
Additionally, the RN will provide condition specific education and empower the enrollee by assistance with coordination of resources and facilitating communication within the healthcare delivery system. The RN will serve as a resource to the employees helping with general questions and understanding of their healthcare benefits.
The Health and Wellness Center is a multidisciplinary team which may include a Registered Dietitian, Registered Nurse, Clinical Counselor, Physician Extender or Fitness Coach/Personal Trainer. The RN will closely monitor risk stratification, compliance with age/gender appropriate screenings, emergency room and urgent care utilization and develop and implement strategies for enrollee education and engagement. Communication and collaboration with the Health and Wellness Center team as well as the AultCare Care Coordination team will be a top priority. Some responsibilities may be shared with other personnel.

Anticipate and follow through with both internal and external customer requests to their satisfaction. Seek assistance of others and other departments when needed.
Development of educational materials and Health and Wellness Center information.
Development of strategies for enrollee engagement with the Health and Wellness Center team.
Maintain confidentiality of enrollee and employer information at all times.
Must possess strong communication and presentation skills.
Process Improvement: Continuously reviews, recommends and implements improvement steps, as needed or directed.
Seeks supervisory guidance/approval as appropriate.
Portrays professional image: follows dress code; communicates with internal and external customers in a professional manner, including appropriate verbal and written grammar.
Comply with policies, procedures and code of conduct, Ohio Nurse Practice Act, National Association of Social Workers Code of Ethics, Case Management Society of America Standards, the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and other governing and accrediting bodies
Utilize the CMSA Standards of Practice for Case Management to assess needs, establish realistic goals, and facilitate an appropriate plan in concert with member/family, facility, and other health care professionals while promoting a cost-effective use of resources.
Promotes and demonstrates professional standards to enhance the development of the department.
Practices ethical conduct.
Meets acceptable attendance and punctuality expectations (excluding FMLA)

Education: Graduate of an approved School of Nursing with a minimum of 4-5 years’ experience. Type and setting of clinical experience will be considered in lieu of the minimum 4-5 years’ experience.
Maintains current unrestricted licensure: R.N. as appropriate with CPR certification
Demonstrates willingness to become certified in Case Management or another appropriate professional specialty.
Excellent written and verbal communication skills.
Proficient with Microsoft Office, including Word, Excel, Access and PowerPoint.
Hours of operation are 7:00 AM to 5:00 PM, Monday through Friday, of which an eight-hour shift is assigned. Occasional adjustment of work hours may be necessary to meet deadlines and attend meetings.
Some travel may be required for which you will be required to use your own automobile with mileage reimbursement as applicable
Lunch and break periods must be coordinated with other staff members to maintain adequate office staffing during departmental operating hours.
Sitting/standing/moving about intermittently during working hours.
