    Summary

    This Social Worker is located in the Social Work Service and is not a bargaining unit position. This is a permanent full-time appointment position.

    Responsibilities

    • Completes comprehensive psycho-social assessments of all-era Veterans in the primary care setting.
    • Collaborates with PACT (Patient Aligned Care Team) team members to include MDs, RNs and allied health professionals.
    • Coordinates non-institutional and institutional services including home health aid referrals, nursing home placements, adulty day health care referrals and respite care.
    • Provides education to family members on the needs of caregivers and refer to caregiver support resources.
    • Provides supportive counseling to Veterans and family members coping with adjustment to illness.
    • Offers palliative and hospice care education, including goals of care discussions, end of life discussion, Advance Directive and MOLST, (Medical Orders for Life Sustaining Treatments) education.
    • Assists Veterans and family members in navigating the VA bureaucracy to secure resources and services.
    • Provides crisis intervention including rapid psycho-social assessments, interventions and treatment plans.
    • Communicates regularly with community resources such as OHA, (Office of Healthy Aging), CCAPs (Community Action Programs) and state Veteran’s office.
    • Documents all electronic assessments and interventions.

    Work Schedule: The work schedule will be determined based on the needs of the Medical Center. The Providence VA Medical Center is an acute care facility operating 24 hours a day 7 days a week. The work schedule will be determined based on the needs of the Medical Center and shift changes will be considered based on patient and facility needs.

    Basic Requirements

    Citizenship: Must be a United States Citizen.
    English Language Proficiency: Must be proficient in spoken and written English.
    Education: You must have a master’s degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. You must submit a copy of your transcripts to be considered for this position.
    Licensure: You must be licensed or certified by a state to independently practice social work at the master’s degree level.
    Drivers License: Valid current state drivers license is required.
    GS-11: Requires: In addition to meeting the basic requirements, do you possess one year post-Masters in Social Work (MSW experience in the field of health care social work or in addition to the basic requirement, do you have a doctoral degree in social work from the school of social work, in which may be substituted for the one years of professional experience in a clinical setting?
    Creditable experience includes independently practicing social work in areas such as, inpatient/outpatient medicine, surgery, mental health, neurology, rehabilitation medicine and geriatrics. Must submit copy of transcripts if applicable.
    In addition, the candidate must demonstrate the Knowledge Skills and Abilities below:

    • Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
    • Ability to independently assess the psychosocial functioning and needs of patients and their family members and to formulate and implement a treatment plan, identifying the patient’s problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team.
    • Ability to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. This requires knowledge of human development and behavior (physical and psychological) and the differential influences of the environment, society and culture.
    • Knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
    • Knowledge of psychosocial treatment and ability to independently implement treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals. This requires independent judgment and skill in utilizing supportive, problem solving or crisis intervention techniques.
    • Ability to independently provide counseling and/or psychotherapy services to individuals, groups and families. Social workers must practice within the bounds of their license or certification. For example, some states may require social workers providing psychotherapy to have a clinical level of licensure.
    • Ability to provide consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Ability to provide orientation and coaching to new social workers and social work graduate students. Ability to serve as a field instructor for social work graduate students who are completing VHA field placements.
    • Ability to independently evaluate his/her own practice through participation in professional peer review case conferences, research studies, or other organized means.
    • Knowledge and skill in the use of computer software applications for drafting documents, data management, and tracking, especially those programs in use by VHA.

    The full performance level of this vacancy is GS-11.
    Creditable Experience: Knowledgeable of Current Professional Social Work Practices. To be creditable, the experience must have required the use of knowledge, skills, abilities, and other characteristics associated with the current professional social work practice. the experience or education must be post-MSW degree. Experience and education satisfying this requirement must be active professional practice, which is paid/non-paid employment as a professional social worker, as defined by the appropriate state licensing board.
    Quality of Experience. Experience is only credible if it is obtained following graduation with a master’s degree in social work and if it includes work as a professional social worker directly related to the position to be filled. Qualifying experience must also be at a level comparable to social work experience at the next lower level.
    Physical Requirements: The nature, frequency, and intensity of physical work are sedentary. Work is performed in an office setting. Most activity is through personal, telephonic, and electronic communication; however, walking around the facility, bending and reaching may be required. Infrequent lifting of 10-20 pounds may be required. Frequently the work requires the incumbent to travel by GSA vehicle to community businesses and various local organizations.

    Preferred Experience: Experience and License at the Advanced Practice Level.
